The following forms are for Open University students only. All forms for Cal Poly matriculated students can be found at the Office of the Registrar.

Form	Description
EPaCE Audit Course Form	Use this form to petition to audit an Open University course.
EPaCE Credit/No Credit Grading Option	Use this form to request that an Open University course be graded on a "credit/no credit" basis.
EPaCE Course Repeat Petition Form	Use this form to petition to exceed the course repeat limitation for an Open University course.
EPaCE Refund Request	Email extended@calpoly.edu to request a refund if you have a credit on your Extend Ed account.
Open University Petition to Withdraw	Email extended@calpoly.edu if you would like to withdraw from an Open University course.
